Sustained virological response in HIV/HCV co-infected patients without rapid virological response (RVR) on peginterferon-ribavirin therapy

چکیده
Methods A prospective study in which HIV-HCV coinfected patients were randomized to receive either pegylated IFNa-2a 180 μg/week plus ribavirin (RBV) at two different doses: i) 1000-1200 mg/day (arm A) or ii) 2000 mg/ day plus erythropoietin (EPO) from baseline to week 4 (arm B), was conducted. Patients not achieving RVR prolonged therapy to 48 weeks for G2-3 and to 72 for G1-4. Liver fibrosis was measured using transient elastometry, being liver stiffness values ≥9.5 kPa as reflect of Metavir ≥F3. RBV trough concentrations at week 4 and pharmacogenetic studies including polymorphisms at the ITPA, ENT1 and IL28B genes were also performed. Variables associated to SVR in those not achieving RVR were examined.

IL28B gene polymorphisms and viral kinetics in HIV/hepatitis C virus-coinfected patients treated with pegylated interferon and ribavirin.
BACKGROUND A single nucleotide polymorphism (SNP) upstream of the IL28B gene (rs12979860) predicts sustained virological response (SVR) to peginterferon-ribavirin therapy in chronic hepatitis C patients. There is scarce information regarding the influence of this IL28B SNP on early viral kinetics during therapy, particularly in patients coinfected with HIV, in whom treatment response is lower t...
متن کاملPrediction of Week 4 Virological Response in Hepatitis C for Making Decision on Triple Therapy: The Optim Study
BACKGROUND Virological response to peginterferon + ribavirin (P+R) at week 4 can predict sustained virological response (SVR). While patients with rapid virological response (RVR) do not require triple therapy, patients with a decline <1 log10 IU/ml HCVRNA (D1L) should have treatment discontinued due to low SVR rate. AIM To develop a tool to predict first 4 weeks' viral response in patients w...
متن کاملInsulin resistance predicts re-treatment failure in an efficacy study of peginterferon-α-2a and ribavirin in HIV/HCV co-infected patients.
BACKGROUND & AIMS Few studies evaluated the efficacy of HCV re-treatment and the predictors of response in HIV/HCV co-infected patients. The role of insulin resistance as a predictor of response in this population is unknown. The aim of this study is to evaluate the safety and efficacy of pegylated interferon-α-2a and ribavirin in re-treatment of HIV/HCV co-infected patients, predictors of sust...
متن کاملFluvastatin as an adjuvant to pegylated interferon and ribavirin in HIV/hepatitis C virus genotype 1 co-infected patients: an open-label randomized controlled study.
OBJECTIVES Recent reports demonstrated in vitro the efficacy of fluvastatin in inhibiting hepatitis C virus (HCV) replication and a synergistic effect in association with interferon-alpha (IFN-alpha). In vivo the inhibition of HCV replication by statins has not been demonstrated. We evaluated in this open-label, randomized controlled study the efficacy of fluvastatin as adjuvant to pegylated-(P...
متن کاملRibavirin and abacavir drug interaction in HIV-HCV coinfected patients: fact or fiction?
OBJECTIVE(S) To examine the impact of ribavirin and abacavir coadministration on hepatitis C virus (HCV) virological response and trough ribavirin plasma concentration (Cmin) in HIV-HCV coinfected patients. DESIGN Pharmacokinetic substudy on patients from the ANRS CO-13 HEPAVIH cohort. METHODS Patients receiving ribavirin-pegylated interferon for whom a ribavirin steady state Cmin was prosp...
